This remains necessary work. Yet the latest data from Britannia P&I reveal a far more insidious threat to seafarers: illness, not injury, is now responsible for the overwhelming majority of deaths at sea. This is not a statistical anomaly; it is a fundamental challenge to how the industry perceives risk, allocates resources, and values the human engine of global trade.

Britannia’s new Crew Claims Report—drawn from nearly 3,000 claims across the 2023/24 and 2024/25 policy years—paints a damning picture. A staggering 81% of crew fatalities were illness-related; only 19% resulted from injuries. Cardiovascular conditions alone accounted for 58% of those illness deaths. This sharpens the club’s broader claim statistics, where illness represents 60% of all crew claims, injuries 36%, and fatalities just 4%. These findings are not an outlier. Shipmanager Anglo-Eastern has similarly reported that illnesses and injuries dominate its active P&I portfolio. The data converge on an uncomfortable truth: for today’s seafarers, the greatest danger is not the ship or the sea, but the human body under prolonged operational stress.

None of this diminishes the reality of workplace hazards. Hand and finger injuries alone made up 27% of Britannia’s injury claims, and more than one-third of injury-related deaths involved man-overboard incidents. These demand continued vigilance regarding safe working practices and procedural discipline. But when four-fifths of deaths stem from illness, the industry’s mental model of “maritime safety” is dangerously incomplete. As Britannia’s Jacob Damgaard notes, viewing safety primarily through the lens of accidents obscures a far larger reality.

Several factors amplify this medical risk. Seafarers work in isolated environments with limited access to advanced care, where common conditions escalate because treatment is delayed. Privacy is scarce, discouraging younger crew in particular from seeking early help. In hotter climates, dehydration-related kidney stones have emerged as a notable problem within the broader category of abdominal conditions. Cardiovascular disease—the leading killer—is fueled by poor diet, chronic stress, irregular sleep, limited exercise, and the cumulative toll of long contracts. These are manageable risks if addressed systematically; left unattended, they become fatal.

A second, quieter crisis is equally troubling. Suicides and missing-at-sea cases disproportionately affect younger age groups. Though absolute numbers remain relatively small, Britannia observes this trend consistently across a wider four-year dataset. This points to mental health pressures that traditional safety systems are ill-equipped to detect or mitigate. Isolation, fatigue, family separation, and the psychological weight of hierarchical shipboard life create vulnerabilities that medical statistics alone cannot capture. When young seafarers are overrepresented in these tragic outcomes, the industry must confront whether its welfare and support structures are fit for purpose.

The policy implication is unambiguous. Accident prevention cannot be abandoned, but it can no longer dominate the agenda. Britannia rightly argues that these numbers make a compelling case for shifting greater attention toward cardiovascular screening, everyday health management, and mental wellbeing. Practical steps follow logically:

· Expand medical screening—pre-employment and periodic—with rigorous emphasis on cardiovascular risk, moving beyond the minimum fitness standards that too often function as a checkbox exercise.

· Improve onboard health literacy and access to confidential medical advice, including telemedicine capable of addressing both physical and mental health concerns without stigma.

· Address environmental contributors—hydration, nutrition, and heat stress—especially on vessels operating in demanding climates.

· Treat mental health as a core safety issue, not an optional welfare add-on, with particular attention to younger crew who appear most vulnerable to extreme outcomes.

· Integrate illness and mental health data into loss-prevention programmes with the same rigor historically reserved for slips, trips, and machinery incidents.

Critics may argue that medical issues are individual rather than operational, or that the industry already meets flag-state and ILO medical standards. Those standards, however, were never designed to reverse the dominance of cardiovascular disease or detect emerging mental health crises among younger seafarers. Claims data of this scale demonstrate that the status quo is insufficient. Prevention is both a moral obligation and a commercial necessity: illness claims already form the majority of the portfolio, and preventable deaths impose human, operational, and insurance costs that the industry can no longer treat as inevitable.

Global shipping depends on seafarers who accept long periods of isolation and elevated risk in exchange for their livelihood. When the leading cause of death among them is illness—rather than the hazards of the profession as traditionally understood—the industry has a duty to respond with equal seriousness. Maritime safety must evolve: from the prevention of accidents to the active protection of health and wellbeing.

The data from Britannia do not merely describe a problem; they demand a change in priorities. Four in five crew deaths are already telling us where the real danger lies. The question is whether the industry is prepared to listen.

<div
